In the Phoenix Guides it is some times said “Update” the file and a listing with some lines marked with - or + in the left margin is given.
Does this mean that I should remove and add respectively the lines?
Or is there a mix verb that does this?

It is unified format of representing diffs between source files.
Git uses this format for generating diffs/patches. In GitHub, you can see the same when you are looking at what changed between commits or pull requests.
