Couple things i’d explore.
Do you have an EXPLAIN query handy?
is there an index? like this?:
create index("stream_events", [:stream_id, "id DESC"])
when you issue the order by it might spill to :memory:?
Something to toggle to see if it narrows down, have it spill to disk instead:

# force temp tables sorting buffers to disk
