so I can conclude that this case is not caused by the bug that perhaps exists from my controller part.

From another topic discussion I found a link to this one(posted by @axelson) and there Cowboy may be the culprit:

Cowboy handlers proably exit, not being logged or reported anywhere

I have investigated this a bit, the issue is here: Cowboy is not handling a {'EXIT', Pid, Whatever}, which is the format of exit reasons from linked processes. I would open up an issue on Cowboy and ask if they would consider adding a catch clause.

You may want to check if it relates with your problem or not.
